We are honored to work with you, and want to make your first visit as pleasant as possible. Please arrive at least 20 minutes ahead of your scheduled time so that you can fill out an Intake Form and other required paperwork. Please be sure to bring a photo ID and your insurance information. If you would like to save yourself some time, you may download the forms by clicking the links below. Print them and fill them out at home, then just bring them with you for your first appointment and arrive 5 minutes before your time.
